Choose Life Car Tags

You can replace your current tag with the Choose Life specialty tag at any time - you do not have to wait until your tag comes up for renewal! You can access a Choose Life specialty tag license plate application form by visiting the Georgia Department of Revenue.

Choose Life tags are listed under "Special Interest Plates"

You may review the Choose Life tag as well as print an application form

Take the completed application form as well as any other required documents to your local Tax Commissioner's office.

If the tag is in your county's inventory and you meet all the requirements for issuance, the tag can be issued to you at that time. If the tag is not in your county's inventory, a temporary operation permit can be issued for your use in operating your vehicle until the tag becomes available.
